The provisions of Section 57 of the Local Government (Miscellaneous Provisions) Act 1976 are applicable to information given in this application and supporting documents (including the proprietorship form) and as such making false statements in giving such information may result in prosecution with a maximum fine equivalent to level 3 (£1000) on the standard scale.

By submitting this application, as vehicle proprietor, you acknowledge and agree that you have read and understood the statement above, including the provisions of Section 57 of the Local Government (Miscellaneous Provisions) Act 1976 and the privacy notice and relevant Hackney carriage conditions of licence or Private Hire Vehicle conditions of licence.
